Giving your consent
Please complete the form below to give your consent for CLAPA to use media featuring you (and/or your child) in places like our website or social media channels.
You should only complete this form if you’ve been asked to do so by a CLAPA staff member. This will usually be for a specific reason (e.g. you’ve sent us some media we’d like to use), but you may also be asked to give ‘general’ consent (e.g. if you or your child are volunteering with CLAPA). You can specify what you’re giving consent for in the form below.
How we use your media
CLAPA has strict policies in place around how and why we use media (such as photos and videos) featuring people from the cleft community. The key points are:
- The safety of the people featured is our top priority.
- We might need to make small edits to what you send us, e.g. removing the names of schools or tweaking a sentence to make it easier to understand.
- If we use these on our website, we’ll phase them out after five years or will seek renewed consent to keep actively using them

If you change your mind
You can get in touch at [email protected] any time to ask to change or remove any media you appear in, no questions asked. We will do everything we can to remove it from our platforms and materials, but keep in mind others may have already saved or shared it.
If you’re in a piece of media submitted by someone else, you can also contact us at any time to ask us to remove it.
